2026 Immunisations

HPV’s/Boostrix Immunisations will take place at school on the 23/3/2026

All Boostrix and HPV forms need to be returned to school by the 6/3/2026

ALL FORMS are due back to the Health Centre by Friday, 8th May 2026, this allows our PHN team to collate forms and contact parents who have not returned a form to us. H

Please: Return these forms to me on a daily basis, preferably before 10am, so I can collate them here and have them ready for our PHNs to collect from school.

All you need to do is mark if the form has been given out to the student, a dot or tick next to the student’s name, so you know the form has gone home with them. YOU DO NOT NEED TO CHECK THE FORMS! Our PHNs will do this.

Any questions please let me know. The quicker these go out and are returned to me, the better.

HPV Vaccinations will be Week 5, Monday, 18th May 2026 – School Hall starting at 9am.
